ScribeUp

For Australian general practice

The AI medical scribe built for general practice.

ScribeUp listens to the consult and writes the note, SOAP or a Chronic Disease Management plan. Nothing you record is ever stored.

Chronic Disease Management

J.K. · 12 Aug · 15 min

GPMP

Hypertension, chronic disease management

Home BP readings averaging 135/85, improved from prior review.

Plan

Diabetic cycle of care

HbA1c, foot check and eye check items scheduled per Medicare requirements.

How it works

Four steps, and you only do two of them.

  1. 01

    Get consent, then start recording

The app asks for the patient's consent before every consult. No consent, no recording.

  2. 02

    It listens while you consult

Captured securely on your own device for the whole consult — a dropped connection doesn't cost you the note.

  3. 03

The note writes itself, in the right structure

SOAP or a Chronic Disease Management note — pick the template the consult calls for, and it's structured the way that document actually needs to read.

  4. 04

You read it, change it, export it

It's a draft and you're the clinician. Edit anything, then copy it into your practice software — the Chrome extension keeps the recorder open in a side panel next to it, so there's no tab-switching.

We can’t leak what we never kept.

Audio, transcript and the generated note exist for exactly as long as it takes to write the note, then they’re gone — not archived, not held for review, gone. The same promise as every ScribeUp product.

Every consult goes through an explicit consent step first, and everything is processed and stored in Australia. Read the full detail on the security page.

A lean set, built for how GPs actually document.

Pick a template per consult, from a quick SOAP note to a Medicare-aligned Chronic Disease Management note. Every tier includes every template.

  • Standard

No fixed structure — the model organises the note using clinical judgement.

  • SOAP Note

Standard consult note, with a tone toggle (Clinical & Formal / Concise Summary / Patient-Friendly) for lighter or heavier visits.

  • Referral Letter

Structured referral to another provider, pulling relevant history from the consult.

  • Chronic Disease Management Note

Combines a GP Management Plan and Team Care Arrangement in one Medicare-aligned document.

  • Discharge/Summary Report

Longer-form summary across visits, generated from the patient's history.

From consult to Chronic Disease Management note, without the typing.

Session excerpt

  • 00:02 You: How's the blood pressure been at home since we adjusted the dose?
  • 00:09 Patient: Better — mostly around 135 over 85 in the mornings.
  • 00:18 You: Good. Let's keep the current dose and review your bloods before the next visit.
  • 00:31 You: I'll also add the diabetic cycle of care items while we're here.

Generated note

  • GPMP

    Hypertension — chronic disease management

Home BP readings averaging 135/85, improved from prior review. Current antihypertensive dose continued. Bloods (U&E, lipids) requested prior to next review.

  • Plan

    Diabetic cycle of care

Added to management plan. HbA1c, foot check and eye check items scheduled per Medicare cycle-of-care requirements.

  • Review

    Next review

6-week follow-up booked; review bloods and BP trend at that visit.

Example for illustration only. Fictional patient, not a real consult.

Fits your EHR

Your practice software stays exactly where it is.

ScribeUp isn’t a practice management system and has no interest in becoming one. Your practice software stays the permanent record. We hand you a finished note and get out of the way — copy it across, or use the Chrome extension to record from a side panel next to the note field you already have open, then copy it across from there.

The billing questions

What counts as a note?
One successfully generated report, counted the moment it's generated, not when you copy or export it. Regenerating or editing the same session's report doesn't count again, only the first generation does. That's the same way the free trial is counted.
How does the free trial work?
Your first 5 notes are free, with no card required and no clock running. Write them this week or over the next two months — the trial ends when you've used the fifth one, not on a date.
What happens if I go over?
Nothing stops. Extra notes are charged at $2.00 each and appear on your next invoice. You'll never be cut off in the middle of a session, and you'll see a warning in the app well before you hit your limit.
Can I change or cancel?
Change tier or cancel any time from your account, effective at the end of the billing period. No lock-in contract, no exit call.

Get your evenings back.

Try it on your next consult. If the note isn’t good enough to use, you’ve spent nothing and you can walk away.
